Medical Authorization:
I hereby grant permission for my child to participate in the FUGO Futsal Summer Camp. I authorize the camp staff to act on my behalf in case of a medical emergency if I or the designated emergency contact cannot be reached. I agree to be responsible for any medical or other charges in connection with my child's attendance at the camp.

Liability Waiver:
I acknowledge that participation in the FUGO Futsal Summer Camp involves physical activity and risks of injury. I hereby release, discharge, and agree to hold harmless FUGO Sports Academy, its directors, coaches, and employees, site facility, site location from any claims arising from injury, loss, damages, or theft that may occur during my child's participation in the camp.

Photo and Video Release:
I grant FUGO Sports Academy the right to photograph and/or record my child in connection with the FUGO Futsal Summer Camp and to use the photography/video for promotional and educational purposes without any compensation to my child or me.

Behavior Agreement:
I understand that all camp participants are expected to adhere to the behavior guidelines set forth by FUGO Sports Academy. I agree that if my child fails to follow these guidelines, the camp staff may take appropriate disciplinary action, up to and including removal from the camp.

Cancellation and Refund Policy:
I understand the cancellation policy as outlined by FUGO Sports Academy. Refunds, if applicable, will be processed according to the terms stated, and I agree to comply with these terms should I decide to cancel my child's registration.

Personal Property:
I acknowledge that FUGO Sports Academy is not responsible for personal property that is lost, stolen, or damaged during the camp and encourage my child to leave valuable items at home.
